Facilities and units are lost whenever the colony is completely wiped out by the plague, regardless of level or event severity. With stock's level 1 plague event limit, you don't see that happen very often due to the small amount of people killed each turn (in the millions or maybe 10m, IIRC). Higher level plagues do not explicitly kill facilities or units; they just kill more people. Once all the people are gone, the colony is destroyed completely.

By hard-coded, I mean the effects of the colony reaching 0 pop are hard-coded.

Event severity has no effect on the event itself; all it does is allow the modder to specify which events can occur with the settings the player chooses. The effects are determined by the values given to the event itself, irregardless of the severity. A particularly cruel modder could make plague level 5s occur with low severity. Wink

The medical facilities work as expected, exactly like they do in stock. You just have to know to expect one turn of plague deaths.

In Adamant, you can only get level 1 or 2 plagues with medium event severity, with level 3s showing up at high severity. Level 4 and 5 plagues are reserved for catastrophic event severity. Sadly, SE4 makes the default severity the absolute highest...

So at Catastrophic you can no longer get L 1, 2 & 3 plagues from events, you mean?

At catastrophic severity, any event can occur. Severity determines the maximum severity of events.
